This form is required for international students who completed the domestic CCCApply application for admission.

Applicants must provide this form to any financial and/or housing sponsor for signatures.

Admitted students can submit this form to accept their offer of admission and save their seat in the upcoming class.

For a travel signature on your Form I-20 allowing you to re-enter the U.S. Complete this form BEFORE you depart the U.S.

Request to drop below 12 units. Allowed for academic difficulty in the first term, medical reasons at any time, or final term.
