Welcome to the Center for Disability & Access (CDA) at the University of Utah. You will need to be a student and have an active uNID and CIS login and password to continue.
If you are a non-student, or a former CDA student who needs to re-establish accommodations, please call us at (801) 581-5020 for assistance.
New CDA Students, please follow these steps:

You can upload documentation via the intake form, or wait to provide documentation at a later time.
2. After completing the registration form, you will receive an email with scheduling instructions within 48-72 hours.
