Our staff provides compassionate care to elderly patients in a variety of settings. Providers and nurse practitioners provide inpatient, outpatient and nursing facility care. We treat patients with memory problems, incontinence, gait instability, fall risks and chronic illness as well as those in need of extended care.

Extended Care
Extended care is for patients residing in subacute care and nursing facilities served by our team. Adams Health nurse practitioners visit patients and provide ongoing medical care as needed. We focus on patients needing rehabilitation after hospitalization and those needing long-term care. We can assist with the coordination of services when our affiliated facilities are used.
Caregiver Support Group
Are you a caregiver of an elder or know of a caregiver who might benefit from education, resources and the chance to connect with others experiencing similar challenges and emotions? We offer monthly caregiver support meetings for individuals caring for elderly loved ones and friends. Meetings take place at Adams Behavioral Health located at
